心力衰竭患者循环内皮祖细胞水平的Meta分析

摘要:

目的:探索心力衰竭(HF)患者外周血中循环内皮祖细胞(cEPC)水平的变化规律。方法:检索PubMed、Embase和Cochrane Library等数据库,同时手工检索南京大学图书馆相关杂志,收集2000年10月至2011年10月发表的关于HF患者cEPC水平的研究。根据纳入与排除标准选择相关研究数据用RevMan 5.1进行Meta分析。结果:共纳入453例患者,166例对照。Meta分析显示总体效应检验P=0.88,HF患者的cEPC水平与正常人的差异无统计学意义。进一步进行分层分析发现,轻度HF组和中重度HF组的cEPC水平差异具有统计学意义,mean(95%CI)为69.24 [12.09,126.39],且中重度HF患者的cEPC水平与正常人的差异具有统计学意义,Z检验P=0.004。结论:中重度HF患者的cEPC水平较正常人的有显著性降低。

Objective: To analyze the level of circulating endothelial progenitor cells (cEPC) in patients with heart failure (HF). Methods: We searched PubMed, Embase, Cochrane Library and journals in Nanjing University library either by computer or by hand. Papers that compared the level of cEPC in HF patients and healthy subjects between October 2000 to October 2011 were pooled, and a formal meta-analysis was performed. Results:There were a total of 453 HF patients and 166 controls. The overall effect showed no difference in cEPC levels between HF patients and healthy subjects. However, stratified analysis showed a significant difference between mild HF patients and moderate to severe ones. 95% confident interval was 12.09 to 126.39. The cEPC level in moderate to severe HF patients was different from healthy subjects with an overall effect of P=0.004. Conclusion: The cEPC level in moderate to severe HF patients is decreased significantly.
