童年创伤对青少年抑郁症患者自伤行为的影响:抑郁和社会支持的中介作用

摘要：

目的: 探讨抑郁和社会支持在青少年抑郁症患者童年创伤经历和自伤行为之间的中介作用。方法: 选取首次在河北医科大学第一医院就诊的214例青少年抑郁症患者作为研究对象,采用一般情况调查表、童年创伤问卷、青少年自我伤害问卷、抑郁自评量表、社会支持评定量表进行问卷调查,并分析童年创伤、抑郁、社会支持和自我伤害的关系。结果: (1)女性青少年抑郁症患者自我伤害评分显著高于男性,差异具有统计学意义(P<0.05)。(2)童年创伤总分与抑郁、自我伤害评分均呈正相关(r值分别为0.256、0.396,均P<0.05);抑郁与自我伤害评分呈正相关(r=0.477,P<0.05);社会支持评分与童年创伤、抑郁、自我伤害评分均呈负相关(r值分别为-0.340、-0.376、-0.523,均P<0.05)。(3)童年创伤总分、社会支持和抑郁评分对自我伤害有预测作用(F=46.972,P<0.001),可解释方差变异的39.3%。(4)抑郁和社会支持在童年创伤与自我伤害之间起到部分中介效应,分别占总效应的20.1%和40.2%。结论: 抑郁和社会支持在青少年抑郁症患者童年创伤经历和自伤行为之间具有双重中介作用。

Objective: To explore the mediating effect of depression and social support between childhood traumatic experience and self-injury behavior in adolescents with depression. Methods: A total of 214 adolescent patients with depression who first visited the First Hospital of Hebei Medical University were selected as the research objects. Using the General Situation Questionnaire,Childhood Trauma Questionnaire,Adolescent Self-Harm Questionnaire,Self-Rating Depression Scale,Social Support Rate Scale,the relationship of childhood trauma,depression,social support,and self-harm was analyzed. Results:(1)The self-injury score of female adolescents with depression was significantly higher than that of male adolescents(P<0.05).(2) The total score of childhood trauma was positively correlated with depression and self-injury scores(r=0.256,0.396,all P<0.05). Depression score was positively correlated with self-injury score(r=0.477,P<0.05). Social support score was negatively correlated with childhood trauma,depression and self-injury score(r=-0.340,-0.376,-0.523,all P<0.05).(3) The total score of childhood trauma,social support and depression score had predictive effects on self-injury(F=46.972,P<0.001),which could explain 39.3% of the variance.(4) Depression and social support played a partial mediating effect between childhood trauma and self-injury,accounting for 20.1% and 40.2% of the total effect,respectively.Conclusion: Depression and social support play a dual role in mediating the relationship between childhood traumatic experience and self-injury behavior in adolescents with depression.
