社区老年人群体力活动水平与疲劳之间的相关性研究

目的:了解南京市社区老年人群体力活动与疲劳之间的流行病学关联。方法:采用多阶段分层随机抽样方法,于2023年7月至12月在南京市12个行政区随机抽取5 556名60岁及以上社区老年人作为研究对象,使用匹兹堡疲劳量表(PFS)测量老年人的疲劳状况;采用国际身体活动量表(IPAQ)中文版短卷评估老年人的体力活动水平;应用混合效应Logistic回归模型计算比值比(OR)及其95%可信区间(CI),分析体力活动与老年疲劳之间的关联。结果:在调查的5 556名社区老年人中,身体疲劳的流行率为58.9%(95% CI 57.6%~60.2%),精神疲劳的流行率为58.4%(95% CI 57.1%~59.6%)。调整年龄、性别、居住地、婚姻状况、受教育水平、吸烟、饮酒、体质指数(BMI)、高血压、糖尿病及其它慢性病、抑郁/焦虑状态和睡眠质量后,与体力活动不足者相比,体力活动充足者身体疲劳的发生风险下降0.29倍(OR=0.71, 95%CI 0.58~0.86)、精神疲劳的发生风险下降0.36倍(OR=0.64, 95%CI 0.53~0.77)。结论:南京市社区老年人群疲劳的流行率较高,充足的体力活动水平与老年人群疲劳之间存在负向的相关性。

Objective: To investigate the epidemiological association between physical activity and fatigue among community-dwelling older adults in Nanjing. Methods: From July to December 2023, totally 5 556 community-dwelling older adults aged 60 years and above were selected from 12 districts in Nanjing municipality with multi-stage stratified cluster random sampling. The Pittsburgh Fatigability Scale(PFS) was used to measure the fatigue status of older adults, and the Chinese version of the International Physical Activity Questionnaire(IPAQ) short form was used to assess their physical activity levels. A multivariate Logistic regression model was applied to calculate the odds ratio(OR) and 95% confidence interval(CI) to analyze the association between physical activity and fatigue among older adults. Results: Among the older adults, the prevalence of physical fatigue was 58.9%(95% CI 57.6%-60.2%), while the prevalence of mental fatigue stood at 58.4%(95% CI 57.1%-59.6%). After adjusting for age, sex, place of residence, marital status, education level, smoking, alcohol consumption, body mass index(BMI), hypertension, diabetes, other chronic diseases, depressive/anxiety status, and sleep quality, individuals with sufficient physical activity had a 0.71-tims risk(95% CI 0.58-0.86) of physical fatigue and a 0.64-tims risk(95% CI 0.53-0.77) of mental fatigue compared to those with insufficient physical activity. Conclusion: The prevalence of fatigue is relatively high among community-dwelling older adults in Nanjing. There is a negative association between sufficient physical activity levels and fatigue among this population.
