家庭亲密度和适应性在老年缺血性脑卒中偏瘫患者自我护理能力与康复锻炼依从性间的中介效应分析

摘要：

目的:调查老年缺血性脑卒中偏瘫患者家庭亲密度和适应性、自我护理能力、康复锻炼依从性的现状,并分析前者在后两者间的中介效应。方法:选取2020年1月至2023年9月我科住院诊治的150例老年缺血性脑卒中偏瘫患者为调查对象,采用问卷调查的方式进行测评。采用Pearson相关分析老年缺血性脑卒中偏瘫患者家庭亲密度和适应性、自我护理能力与康复锻炼依从性之间的相关性,并构建结构方程模型进行中介效应检验。结果:150例老年缺血性脑卒中偏瘫患者康复锻炼依从性评分为(35.37±5.54)分,自我护理能力评分为(77.54±11.39)分,家庭亲密度和适应性评分为(98.61±12.37)分。康复锻炼依从性与自我护理能力、家庭亲密度和适应性之间均呈正相关(均P<0.05);自我护理能力与家庭亲密度和适应性之间呈正相关(P<0.05)。家庭亲密度和适应性在自我护理能力与康复锻炼依从性之间的中介效应值为31.11%。结论:自我护理能力可以通过调节家庭亲密度和适应性间接影响老年缺血性脑卒中偏瘫患者康复锻炼依从性,为临床护理工作开展提供参考依据。

Objective: To understand the current status of family intimacy and adaptability, self-care ability, and rehabilitation exercise compliance of elderly patients with ischemic stroke and hemiplegia, and analyze the mediating effect of the former on the latter two. Methods: 150 elderly patients with ischemic stroke and hemiplegia who were hospitalized in our department from January 2020 to September 2023 were selected as the survey subjects, and were evaluated using questionnaires. The correlation between family intimacy and adaptability, self-care ability, and rehabilitation exercise compliance in elderly patients with ischemic stroke and hemiplegia was analyzed using Pearson correlation analysis. Structural equation model was constructed to test the mediating effect. Results: The rehabilitation exercise compliance score of 150 elderly patients with ischemic stroke and hemiplegia was(35.37±5.54) points, the self-care ability score was(77.54±11.39) points, and the family intimacy and adaptability score was(98.61±12.37) points. There was a positive correlation between rehabilitation exercise compliance and self-care ability, family intimacy and adaptability(all P<0.05). There was a positive correlation between self-care ability and family intimacy and adaptability(P<0.05). The mediating effect value of family intimacy and adaptability between self-care ability and rehabilitation exercise compliance was 31.11%. Conclusion: Self-care ability can indirectly affect the rehabilitation exercise compliance of elderly patients with ischemic stroke and hemiplegia by regulating family intimacy and adaptability, providing a reference for clinical nursing work.
